The most frequent failure is an unclear split of responsibility. When the quality agreement is vague, complaints, CAPAs, and change requests fall through the gap between the two quality systems. Write it in detail before the first production run, and revisit it whenever the process changes.

Change control is the second recurring problem. A CMO that alters a supplier, a resin grade, or a process parameter without telling the client can invalidate a validation and, in the worst case, the device’s regulatory status. Good practice ties every proposed change to an impact assessment and a defined approval route.
